DUTIES AND RESPONSIBILITIES: Partner with other Wellness Navigators and community partners to deliver exceptional client service. Act as a liaison between clients, their treatment team, and other health agencies. Build relationships and trust with the clients to ensure that clients are receiving proper care and treatment in the services they require and must maintain confidentiality at times. Perform client Triage/intakes and assessments to determine needs. Conduct brief telephonic counselling support, as appropriate. Facilitate warm transfers of callers in distress to crisis services or to on-call counsellors. Provide tailored support for clients throughout their lifespan including system navigation, care coordination, communication, and collaboration with the client's circle. of care to ensure best outcomes for client wellness Maintain and update client records in accordance with practice standards. Perform regular client file audits to consistently enable excellence in record management. standards Other administrative duties as assigned. ACCOUNTABILITY: This position reports to the Clinical Services Coordinator COMPETENCIES AND QUALIFICATIONS: Willing to train. Some prior education or training and experience working in mental health related fields is considered an asset. Knowledge and understanding of indigenous culture and values in a regional context. Strong interpersonal and communication skills (written and oral). Must be willing to provide a police criminal background check. Ability to maintain strict confidentiality. Professional experience working with youth is a requirement of this role. Professional experience in Psychology, Social Work/Social Services, Health, Healthcare Administration an asset Bachelor's degree or equivalent in relevant field preferred (i.e., Psychology, Social Work, Indigenous Services, Traditional Counselling, Healing and experience in a Mental Health-related field). Registered Nurse with Mental Health specialty; Minimum of a 2–3-year relevant college diploma or equivalent academic and professional experience preferred (i.e., Social Service Work, Child and Youth Counsellor, Health-related field, Healthcare Administration, etc.) Crisis/distress/de-escalation training and experience Excellent organizational and time management skills Excellent verbal, written and interpersonal communication skills. Exceptional client service skills Demonstrated high computer proficiency, including MS Office (Word, Excel, Outlook, and PowerPoint) Ability to speak Cree, Oji-Cree, or Ojibway is an asset.
